REGISTRATION NOTES:
When you register for your first sport season of the school year, you will need to fill out all the forms listed on the right except for the EL9. These are the SF HEAT Registration Form, and the FHSAA's EL2, EL3, and the EL7.
The Verification Form of the EL7 is the only form you send to the school board. The Registration Form of the EL7 needs to be notarized.
Don't forget to include your transcipts for your high school athletes and a copy of the evaluation that you sent to the school board for middle schoolers** & ninth graders.
**VERY IMPORTANT: If you child is going into the 7th or 8th grade this year, you will need to send in copies of the last 2 years of the evaluations that you sent to the school board.
If your child attended a school last year, you will need to include transcripts from that school whether they are a middle schooler or high schooler.
ALL FORMS are then sent or given to Coach Bob Bemis including the Verification Form when you receive it back from the school board.

EL3 - Consent and Release from Liability Certificate
EL7 - Athletic Participation Registration Form for Home Education Student, Notice of Intent, & Verification Form (It also includes the guide to the EL7 to help you fill out this form correctly. You may want to select the first 3 pages to print unless you want to print the complete document)

The EL9 form is only for athletes who participated in a fall and/or winter sport and are returning to participate in a varsity spring sport.
EL9 - Home Education Student Academic Progress Report - required by athletes that participated during the first semester of the school year and wish to participate again in the second semester
