Sport Season Registration Forms:
REGISTRATION NOTES:
When you register for your first sport season of the school year, you will need to fill out the forms listed on the right. These are the SF HEAT Registration Form and the SF HEAT Agreement Signature Form, the FHSAA's EL2, EL3, EL7, and possibly the EL9. Please see note under the EL9 to determine if you are required to submit this form.
The Registration Form of the EL7 (page 1 of 2) needs to be notarized.
The Verification Form of the EL7 (page 2 of 2) is the only form you send to the school board. We recommend you send a self-addressed, stamped envelope with it.
HIGH SCHOOL STUDENTS:
Don't forget to include your transcripts for your high school athletes. The EL7 Guide has a sample of what needs to be included.
Remember to also attach actual school transcripts from "any" classes taken at a school including dual enrollment classes.
If your child attended a school (was not homeschooled) last year or during any of the high school years, you will need to attach an actual school transcript.
6th, 7th, 8th & 9th GRADE STUDENTS:
When they are asking for a "transcript" for the middle school and 9th grade, they are asking you to fill out the EL9 for the past semester - not a cumulative transcript like the high school students submit - just the grades received for the last semester. If you are a 6th grader entering into the HEAT for a Fall or Winter Sport (first semester, Aug-Dec), you do not need an EL9 form to register.
Remember to attach actual school transcripts from "any" classes taken at a school.
If your child attended a school (was not homeschooled) last year or for the last semester, you will need to attach a transcript from the school.

ALL FORMS are then sent or given to Coach Bob Bemis including the Verification Form when you receive it back from the school board. Please do not staple them together.
